To help you prepare for everything coming up this year, we are partnering with Northside Cherokee to offer FREE sports physicals.

Here's how
1. Select a time slot, and make an appointment.
Anyone under 18 years of age, must have an adult parent or guardian present throughout the duration of the visit.
2. Print, fill out, and sign the 'History & Evaluation' form before arriving.

3. Arrive 15 minutes before your appointment time and report to the check-in stand.

A sports physical is different than the annual physical because it focuses on reviewing your child's current health status and medical history to ensure that your child is healthy enough to play his or her sport.
